Instructions:
- Click on the form name to download/print
the corresponding file (application for graduate study is a large, 2.2
mb file, please be patient). You need to print out three copies
of the recommendation
letter and a single copy of each of the other documents.
- Fill out each field in the graduate
application form carefully.
- Put your name on the recommendation forms
and sign a confidentiality agreement if you choose to waive your right
to access information provided by your referees. Give a copy of
the recommendation form to each of your three references (must be your
instructor, employer, or clinical supervisor) at least a months before
the application deadline and ask them to send their letters directly to
the Admissions Office a week or so before the deadline.
- Prepare a thoughtful, well-written
personal statement. Address each of the suggested issues and
support your answers with evidence where appropriate.
- Preparing a good application takes
time. Start as early as possible. Consult with the
Psychology Department if you have questions about the application
process.
