The "senator application form" is only for special circumstances where there are vacant seats on the senate during mid-year and are only filled out and processed when UHHSA properly promotes this need.

If you are a RISO and would like to request funding please print out and fill out this form. Bring it to the UHHSA office and meet with a senator to discuss funding requests.
