Effective 10/09/06

  1. Delinquency of submission to Business Office
    (transaction log, receipts, bank statement, statement of account)
    1. Warning via e-mail
    2. Warning via e-mail with copy to Supervisor
    3. Temporary suspension of use of Pcard (attend training)
      1. Warning via e-mail
      2. Warning via e-mail with copy to Supervisor
    4. Permanent suspension of use of Pcard
  2. Correction of errors, missing documents, justifications.
    1. Warning via e-mail
    2. Warning via e-mail with copy to Supervisor
    3. Temporary suspension of use of Pcard (attend training)
      1. Warning via e-mai
      2. Warning via e-mail with copy to Supervisor
    4. Permanent suspension of use of Pcard
  3. Inappropriate/unallowable purchase/parceling
    1. Warning via e-mail with copy to Supervisor and reimbursement to UHH.
    2. Temporary suspension of Pcard use & reimbursement to UHH (attend training)
      1. Warning via e-mail with copy to Supervisor and reimbursement to UHH
    3. Permanent suspension of use of Pcard and reimbursement to UHH
  4. Neglect of updating PVSnet
    1. Warning via e-mail
    2. Warning via e-mail with copy to Supervisor
    3. Temporary suspension of use of Pcard (attend training)
      1. Warning via e-mail
      2. Warning via e-mail with copy to Supervisor
    4. Permanent suspension of use of Pcard
  5. Misuse
    1. E-mail notification with copy to Supervisor and Vice Chancellor for Administrative Affairs AND permanent suspension of use of Pcard AND reimbursement to UHH

Note: If any problem is not resolved, Pcard holder must reimburse the University.
